{"id":702263,"date":"2026-05-14T00:00:00","date_gmt":"2026-05-14T00:00:00","guid":{"rendered":"https:\/\/enterpriseam.com\/egypt\/?p=702263"},"modified":"2026-05-14T12:24:41","modified_gmt":"2026-05-14T12:24:41","slug":"why-mental-health-concepts-struggle-to-translate-in-egypt","status":"publish","type":"post","link":"https:\/\/enterpriseam.com\/egypt\/2026\/05\/14\/why-mental-health-concepts-struggle-to-translate-in-egypt\/","title":{"rendered":"Why mental health concepts struggle to translate in Egypt"},"content":{"rendered":"<p style=\"padding:0;margin:0;line-height:1.15;orphans:2;widows:2;text-align:left\"> \ud83e\udde0 <strong>May marks <\/strong><strong><a target=\"_blank\" href=\"https:\/\/enterpriseam.com\/egypt\/2026\/05\/01\/your-mental-health-is-a-priority-a-guide-to-essential-support-centers-and-digital-platforms\/\" style=\"\">mental health awareness month<\/a><\/strong><strong>, observed <\/strong><strong>annually across the US. <\/strong>Though not officially recognized in Egypt, the month\u2019s significance still resonates locally as Western mental health terminology continues to seep into our everyday language, raising the question: where does the translation begin to fail \u2014 both linguistically and culturally?<\/p>\n<p style=\"padding:0;margin:0;line-height:1.15;orphans:2;widows:2;text-align:left\">\n<p class=\"tag-border-left\">What has changed in Egypt\u2019s mental health sphere?<\/p>\n<p style=\"padding:0;margin:0;line-height:1.15;orphans:2;widows:2;text-align:left\"><strong>Egypt\u2019s mental health landscape has long been plagued by stigma, myths, underfunding, illiteracy, high costs, and more. <\/strong>While it\u2019s difficult to say public attitudes toward mental illnesses have done a 180 over the past 10-20 years, it\u2019s fair to say that many changes are taking shape.<\/p>\n<p style=\"padding:0;margin:0;line-height:1.15;orphans:2;widows:2;text-align:left\"><strong>In Om el Donia, mental health care largely falls under the umbrella of the Health Ministry <\/strong>and its specialized mental health arm, the General Secretariat for Mental Health and Addiction Treatment (GSMHAT). The <a target=\"_blank\" href=\"https:\/\/www.expatfocus.com\/egypt\/guide\/egypt-mental-health\" style=\"\">2009 Mental Health Act<\/a> focused on protecting the human rights of psychiatric facility patients \u2014 legislation aimed to \u201cestablish [regulatory] frameworks, combat stigma, raise public awareness, and protect patient rights,\u201d according to Expat Focus.<\/p>\n<p style=\"padding:0;margin:0;line-height:1.15;orphans:2;widows:2;text-align:left\"><strong>Fast forward to 2026, and the Health Ministry announced the \u201c<\/strong><strong><a target=\"_blank\" href=\"https:\/\/sis.gov.eg\/en\/media-center\/news\/health-ministry-your-health-is-happiness-initiative-boosts-mental-well-being-nationwide\/\" style=\"\">Your Health is Happiness<\/a><\/strong><strong>\u201d initiative<\/strong>, with the aim of providing integrated services such as dedicated hotlines, early detection programs, and training programs for healthcare workers. The move introduced psychological services into primary healthcare units for the first time.<\/p>\n<p style=\"padding:0;margin:0;line-height:1.15;orphans:2;widows:2;text-align:left\"><strong>Despite gradual progress, mental health remains stigmatized<\/strong>, often seen as a flaw that can stand in the way of marriages, jobs, relationships, and more.<\/p>\n<p style=\"padding:0;margin:0;line-height:1.15;orphans:2;widows:2;text-align:left\"><strong>Social psychologist and Adjunct Professor of Psychology at AUC <\/strong><strong>Hala Mahmoud (<\/strong><strong><a target=\"_blank\" href=\"https:\/\/www.linkedin.com\/in\/hala-w-mahmoud-ph-d-66b05b74\/\" style=\"\">LinkedIn<\/a><\/strong><strong>) sees improvements in mental health awareness. <\/strong>She largely attributes this shift to defining moments over the last 15 years \u2014 most notably the 2011 revolution and the expanding reach of social media during that period, particularly as it became accessible to \u201cdifferent segments of the population,\u201d Mahmoud tells EnterpriseAM. The revolutions sparked a widespread interest in social media engagement and previously taboo subjects in general, exposing Egyptians to conversations around mental health led by specialists both within and outside of Egypt.<\/p>\n<p style=\"padding:0;margin:0;line-height:1.15;orphans:2;widows:2;text-align:left\"><strong>This emerging circulation of information played out as a double-edged sword. <\/strong>Mahmoud notes that it drove a greater demand for mental health support, as rising social media use became a catalyst for mental health struggles among younger adults. While social media played a role, she also acknowledges that economic crises and political instability were contributing factors to increased vulnerability to mental health challenges.<\/p>\n<p style=\"padding:0;margin:0;line-height:1.15;orphans:2;widows:2;text-align:left\"><strong>The exposure raised awareness, yes, but what was lost in the cracks of an ill-suited system?<\/strong> As we increasingly looked outward for psychological guidance, much of it seemed like a saving grace at first. Eventually, it became clear that mental health frameworks are far from one-size-fits-all.<\/p>\n<p style=\"padding:0;margin:0;line-height:1.15;orphans:2;widows:2;text-align:left\">\n<p class=\"tag-border-left\">Language and culture at play<\/p>\n<p style=\"padding:0;margin:0;line-height:1.15;orphans:2;widows:2;text-align:left\"><strong>The disparity begins with language itself.<\/strong> Mahmoud points to how the word \u201cpsychology\u201d carries different meanings and connotations than its Arabic translation, \u201cilm al nafs.\u201d A heavy term like \u201ctrauma,\u201d for instance, also carries very different connotations across both languages. Its Arabic translation \u201csadma nafseya\u201d often emphasizes the idea of \u201csadma\u201d \u2014 suggesting \u201cit\u2019s a thing caused by a one-time event, an accident, or something horrible happening to a person once,\u201d Mahmoud explains. This simplistic impression can obscure the fact that trauma is multifaceted and comes in various forms, including chronic, acute, complex, micro, and macro trauma, she adds.<\/p>\n<p style=\"padding:0;margin:0;line-height:1.15;orphans:2;widows:2;text-align:left\"><strong>Beyond the obvious linguistic gaps<\/strong>, values and cultural concepts face an even greater struggle in translation \u2014 enter the classic collectivist-versus-individualist divide. Mahmoud emphasizes that ideas surrounding \u201cwhat it means to be mentally stable and healthy and live a fulfilling life\u201d take on very different meanings here in Egypt.<\/p>\n<p style=\"padding:0;margin:0;line-height:1.15;orphans:2;widows:2;text-align:left\"><strong>This mismatch between imported therapy models and local realities has long been well documented. <\/strong>A 2012 <a target=\"_blank\" href=\"https:\/\/journals.sagepub.com\/doi\/10.1177\/0020764012452349\" style=\"\">review<\/a> of 22 psychosocial or mental health studies in MENA countries found more barriers (68%) than promoters (32%) when it comes to effectively translating and adapting psychosocial interventions.<\/p>\n<p style=\"padding:0;margin:0;line-height:1.15;orphans:2;widows:2;text-align:left\"><strong>When language and frameworks fail, the effects often manifest physically<\/strong> \u2014 such as the common local struggle with \u201cal qawlun al asabi,\u201d or irritable bowel syndrome (IBS), which is a frequent symptom of anxiety disorders. \u201cBecause of the stigma, and because many segments of the population do not have the language to express these struggles, they get expressed through physical symptoms, and people end up seeking medical help from physical doctors,\u201d Mahmoud says.<\/p>\n<p style=\"padding:0;margin:0;line-height:1.15;orphans:2;widows:2;text-align:left\">\n<p class=\"tag-border-left\"> The clash<\/p>\n<p style=\"padding:0;margin:0;line-height:1.15;orphans:2;widows:2;text-align:left\"><strong>\u201cVery common on social media are the ideas that \u2018happiness is your responsibility\u2019 and 'you choose to be happy\u2019 <\/strong>\u2014 these are very Western models of thinking about happiness or well-being,\u201d Mahmoud tells us.<\/p>\n<p style=\"padding:0;margin:0;line-height:1.15;orphans:2;widows:2;text-align:left\"><strong>It\u2019s hard for traditional therapy approaches to adopt these ideas in Egypt<\/strong>, because they contradict very prominent cultural values that prioritize family and community over self-fulfillment. Mahmoud points to common situations in which someone may struggle to pursue their future or full potential because they, for example, have elderly parents who need their help and care. In cases like these, encouraging a client to prioritize personal fulfillment can conflict with cultural expectations, she explains.<\/p>\n<p style=\"padding:0;margin:0;line-height:1.15;orphans:2;widows:2;text-align:left\"><strong>Therapists in Egypt, who are typically trained on Western concepts, have to navigate these cultural sensitivities carefully.<\/strong> One of the first guiding principles of mental health and psychosocial support is \u201cdo no harm\u201d \u2014 meaning that, even with good intentions, therapists must remain mindful not to harm clients by overlooking the cultural and social values shaping their lives.<\/p>\n<p style=\"padding:0;margin:0;line-height:1.15;orphans:2;widows:2;text-align:left\"><strong>For something as widespread as managing anxiety disorders<\/strong>, she says it\u2019s vital to ask questions like: \u201cHow does it apply locally? How do people in the local culture experience anxiety? How do they express it? What kind of help-seeking behaviors are common? And so on.\u201d<\/p>\n<p style=\"padding:0;margin:0;line-height:1.15;orphans:2;widows:2;text-align:left\"><strong>It\u2019s not just cultural nuance <\/strong>\u2014 Mahmoud, who works directly with refugees in NGOs in Egypt, sees firsthand how people often live in circumstances beyond their control. \u201cThe economic crisis, especially among the lower socio-economic groups, does affect people\u2019s mental health,\u201d Mahmoud adds.<\/p>\n<p style=\"padding:0;margin:0;line-height:1.15;orphans:2;widows:2;text-align:left\"><strong>Is it all bad, though? Fortunately not.<\/strong> The bigger picture includes not only \u201cstressors\u201d but also \u201cprotective factors,\u201d according to Mahmoud. \u201cThe communities and families are very tight-knit. Most people, not all of course, are connected to a community, and this is something that is very much lacking in other, especially Western, cultures,\u201d he says.<\/p>\n<p style=\"padding:0;margin:0;line-height:1.15;orphans:2;widows:2;text-align:left\"><strong>\u201cUnfortunately, the therapeutic model that is prevalent in Egypt does not take this into consideration, mostly focusing on the individual.<\/strong> But if you bring in the entire family and the social context around the individual, the individual will have much better prospects at recovering from whatever challenges or disorders they have.
